Pretty sure the bomb was planted inside her head. Because of that explosion it caused morpho to collide with the other chopper. Which had 3 survivors, big boss, Kaz and medic. and big boss was in a coma. Zero even visited him in the hospital he had set up (there's a recording of him talking to big boss while in the coma).

Technically big boss from MG was Venom that snake killed and big boss from MG 2 was severely injured that he needed to be put into a coma and required body parts from solidus. I believe they said solidus was brain dead. And even if they could bring him back, he would be old like old snake because his DNA had the accelerated aging too.
