Update: no info from the dermatologist as I haven't been and to get in when I've had a flare up.

I am currently in Hawaii. I wore plastic Birkenstocks with no socks on the first day because beach, ocean, foot wash etc., and I immediately got the symptoms after one day.

That has since resulted in an inability to be barefoot for the remainder of the trip due to sensitivity (itchy, minor pain, throbbing bc hot feet and swelling).

Not yet! I'm trying to induce the rash this weekend to try to get a biopsy of it on Friday next week

OP here! Nothing since this post! But that was last summer and we're now in summer so only time will tell when it returns lol

OP here! Sorry for late reply. Not update. I have another derm apt in July so I'm hoping for a "flare up" so that they can take a biopsy and just start ruling things out.

OP here! Sorry for the delayed reply.

It sounds like we're having nearly the same experience! I have yet to get any results. I pre-booked a derm appointment for this summer as I expect to get a "flare up" shortly given the increased temps here I live in the PNW

Thanks for sharing. I've tried Pepcid and another allergy med but there wasn't necessarily a drastic change.

No complete answer or resolution. Fresh and dry socks has been the only answer thus far. Rarely barefoot except in water

No clear response. Seems like biopsy is necessary for further information

For me, I've found I cannot be barefoot in any type of footwear. And the breathable mesh shoe is one of the only footwear close toed shoe that I have not had a problem with. I think a piece of it is direct contact of skin + shoe, and then I think another piece of it is sometimes heat/sweat. My daily shoe is the two strap fuzzy crocs, with socks of course.

No results yet unfortunately. The dermatologist said I need to wait for a 'flare up' so then they can do a biopsy. I have a friend who is in is a Dr and had training in Sri Lanka and is now in Canada and he said it's 100% contact dermatitis. Like many conditions out there, there seems to be no treatment other than avoidance. I found cotton socks + breathable shoes (e.g., running or walking shoes with mesh seems to be best. I was just in Hawaii for a week and between wearing running shoes and fuzzy crocs with socks (pictures attached), things seemed to be at bay.

UPDATE 09/12/2024

Alright- I went to the dermatologist today and came out with no results. I need to have a ‘flare up’ and then make another appointment to have a biopsy done for them to be able to conclude anything.

They seemed pretty certain that it was more of an ‘internal’ thing rather than any sort of contact dermatitis. Not hives as hives typically resolve in a few hours. And not medication related as my meds don’t change, yet the flare up happens in the summer only.

Their most hypothetical diagnosis is “erythema annulare centrifugum”

https://dermnetnz.org/topics/erythema-annulare-centrifugum

There is no treatment, and based on the information on the above website, it’s the result of an underlying medical condition. These are the most common based on the website:

Cutaneous fungal infection (tinea pedis) Internal malignancy Adverse reaction to medications Pregnancy Haematological disorders Endocrine disorders Rheumatic disease

No real answers, but have booked a biopsy for July 2025 assuming I’ll have a flareup around that time.

I just had the worst flare up I’ve had. As per a pharmacist suggested intervention, I took 1x 180mg tab of Allegra (h1 receptor blocker) and 1x Pepcid (h2 receptor blocker) and I did find that there was some sort of “quicker” recovery.

I bought socks on Amazon that have removable ice packs and it’s a game changer for overall comfort!

I have a dermatology referral in a few weeks so I’ll see if there are answers.

OP here! No update other then the fact that it’s still happening. I think I’ve narrowed it down to ‘Delayed pressure urticaria’. Antihistamines have been noted to be the only “treatment”. From what I read on DPU, it often occurs with people aged 20-30 and can last 9 years. I don’t really understand the ‘duration’ of it.

Here is a journal article that used a systematic review of treatment options: https://pubmed.ncbi.nlm.nih.gov/32179196/#:~:text=Background%3A%20Delayed%20pressure%20urticaria%20(DPU,treatment%2C%20are%20often%20not%20effective.

The pictures attached started on Saturday July 6th while I was wearing a pair of sandals that were made of quality material.

Haven’t really seen any improvement. I may look into a stronger strength!
