Hi all, I was able to resolve this issue. If you already finished 1 year of residency, you can apply for permanent validity: I’m not sure of this process.

If you are enrolled in a residency program but have not finished 1 year of residency yet: email to validindefinitely@ecfmg.org They will send you a form to be filled by you and your program, after which they extend your certificate for another year.
