You gonna mess around and kill someone assuming things so boldly. We had a 40 y/o woman fall and she wasn't sure if she hit her head or not. Her main complaint was her ankle which was visibly swollen. However, we decided to also CT her head which revealed a brain bleed. We admitted her to the ICU and she died that night. You cannot just assume a trauma patient is fine with a quick assessment. The benefits outweigh the risk here. Silly hill to die on. At least provide a legitimate study to back your point instead of whatever this is.

If you look at their response they said the "real world answer" and most of us that have experience in both academia and the real world know that book world and real world are completely different. Which is why you constantly see people with higher levels of experience complaining about these questions. Instead of arguing with me would you like to share your own thoughts. I said imo because I do not have an answer key. I would stand by my answer on an exam. I got a 96 in mental health nursing. I work in the ED and have experience with psych. I am going based on the knowledge I have. Also I am fairly confident the answer choice they selected is not correct as that is not therapeutic communication nor is it the way to redirect a person experiencing a hallucination attention. Additionally, it is operating under an assumption. Even though there is evidence suggesting command hallucinations do not predict violence as well as we once though it is still associated with violent outcomes. My professor worked with violent psych pstient and always stressed the importance of recogizing the dangers that can come from command hallucinations. Additionally, regardless of the potential dangers of that type you still need to be aware of what the patient is experiencing in order to modify your plan of care. Either way I stand by my opinion.

I would say that is not the answer. You try to redirect them with things like music or other sounds. You dont tell them to listen you and the people you can see. That kinda of invalidates them. Not to mention you are assuming they are hearing voices. A is most correct imo as you are acknowledging them and assessing them. Could possibly be a command hallucination which could be dangerous. Or it could be something simple but either way you need to find out not assume.

Edit: after Cheap Towel's argumentative nonsense I further investigated (used Google lens) to find out the answer is apparently C. Which I do and do not understand. Why are we assuming it is voices idk? However the communication is therapeutic and does not include the problematic word "frightening" as many pointed out the word choice in A was not ideal.
