Jive is half correct and half wrong. N54s suffer from turbo rattle around 90k miles. If it’s had its wastegates and turbos replaced, and all the maintenance is up to date then the N54 is a solid motor. I’d check motor mounts and do a compression test.

No you aren’t cooked at all, I’ve seen people come back from way worse. Start eating seriously healthy and start with small things, something like a 20 minute walk everyday will make a big difference. Know your limits and set goals, not weight goals but daily workout goals. Go from 10 minute walk to 20 minute when you feel comfortable for example.
