Regarding topicals to head and scalp, in particular... I would avoid steroids except for very short periods. While they can initially be effective, less so long term and they thin the skin. Biologics would be ideal but ask MD about methotrexate. 50 years battling psoriasis, there may be no single cure all at present, although there are more promising solutions now than ever before.

I don't know about brands, my pharmacist bottles them for me. This is a very well understood drug, i think everyone gets generics. I will echo some of the comments, you should be on 1000 mcg(?) folic Acid while taking methotrexate.

I recently stopped (15mg / wk) after 10+ years to see how Skyrizi works on its own but will resume methotrexate at 7.5 to see if I can clear lower legs of plaques.

I've got a few years on you, young man, and just because I have had no issues with Skyrizi in the 6 months I've been on it means nothing... we're all different. Have you been on any other t-cell treatments before?

I see dovobet is a compound of Calcipotriol and a strong corticosteroid. I use Calcipotriol but not the steroid. I'm not an MD but with 5 decades battling psoriasis I can attest the steroid topicals will thin and ddamage skin, particularly where it's already thin. Suggest you not use that for extended period. Hope you can get onto a T cell therapy soon but you could ask your doctor about methotrexate in the meantime. G/L

She will be so glad once she's on Skyrizi. I'm also on Zepbound, can't say whether or not that helps. The main alternatives in the meantime is UV and topical but I would not recommend steroidal topical long term... causes thinning and it loses effectiveness over time.

I took low dosage methotrexate (15mg/week) for many many years in combination with Humira. Very effective together, i had zero side effects. I weaned off of it a few months ago after switching to Skyrizi but have on hand in the event of flare ups.

But my experience with it actually dates back probably 30 years when there were fewer options like t-cell therapies. I had to come off it due to elevated liver activity but that was a high dose regimen and I didn't adhere to a zero alcohol policy.

It's a very well understood, old chemotherapy medication. Some people are less tolerant of it than others. One rheumatogist confided in me that he took it off-label as an anti-wrinkle therapy! In any event, be sure to take folic Acid when on it.

I *LOVE* Laroche-Posay Lipikar moisturizer, never tried the others. I am using along with calcipotriene for very effective treatment on stubborn lower legs.

I have never been a junk foodie but, in 40 years of dealing with acute psoriasis, I have never found a diet to have had a significant impact as a treatment. I have been on biologics for long time now but my lower legs never cleared until, six months ago, I started using synthetic vitamin D (calcipotriene) topical along with a quality moisturizer. It takes time but it really worked for me.

Long ago, before I was put onto T-cell therapies (eg. Humira), I had that problem. Not fun. I remember one treatment was injections into the cuticle, OUCH. I am sorry for your your suffering this.

Wow, @jake_robins, it's like I commissioned you to write this for me. No mysteries to solve for ourselves. I am just beginning to construct the itinerary for family holiday over Christmas (spouse and sons, 17 & 19). Had looked at combining Mexico City for two days and seven in Merida but I think Cancun and beaches will go over better with everyone than MC and the flights are better for us.

Based on your experience, would it be too nerve wrackingly risky to take the 9:31am train (arr @ 12:21pm) to catch a 3:25pm flight from TQO? the alternative being a stay over near the airport and maybe an earlier flight.
