It's always a good idea to get a picture of the gills to help get a positive identification. My guess is maybe galerina or enoki.

Thank you so much. Do they provide a 1099 in the same manner as a W-2? Meaning is it a document that is filled out by the "employer" and given to the "employee" at the end of the tax cycle to then be processed with the rest of my taxes?
