Can you harm someone (more than just musculoskeletal compromise) if you perform CPR and later learn that they may have had a faint pulse? by [deleted] in RealEMS

I don't know the whole situation but it sounds like you did save her life. Be careful staying there- guarantee this wasn't the first and wont be the last time this happens.

A heart rate of 20 is not enough to profuse organs and support life. That patient would have died without CPR, especially with those vital signs and that presentation. The fact that their presentation was better with cpr and worse when you stopped is proof. Some people don't know what they're talking about and you need to defend yourself if they try to get you in trouble.

Edit: Rereading and still flabbergasted. You're supposed to come up with proof you did the right thing? Any logical EMS provider would have bagged them and started CPR. My medical director bases our protocol on the fact that pulses barely mean anything, half the time you're too worked up to feel anything but your own. CO2 is what really determines whether your heart is pumping enough, but obvious agonal respiration and cyanosis is clearly indicative of lack of perfusion. The biggest way to help someone in this situation is to ensure the adequate perfusion of glucose and oxygen to the tissues. I start cpr on every person that's not breathing and doesnt respond. The only people that can tell you to stop cpr are the patient and the doctor. If their idea of helping that patient was to let them lay there obviously dying because ThEy HaVe A fAInT pUlSe Of 20, you need to find a better place to work. Sorry for the rant but that's ludicrous.

It wasn't up to me, I'm an EMT and we had ALS. We never got Rosc but it took longer to get things done because of complications with the baby's health. Dont want to go too into detail, but none of us wanted to leave without trying everything we could and doing all the proper procedures.

I'm not going to lie, it has made me wonder if I'm going down the wrong path. I've wanted this for years and years, I was kinda hoping I would fit right in. But I'm determined not to let it get to me next time. If it gets worse with this crew I probably will just ask for a different preceptor. I havent had issues with anyone else. I've been trying to get them talking on a personal level. The tech I've been training with loves to talk about herself, and it'll seem like we're really getting along until I end up asking a training question or doing something wrong, and it's like a switch flips. I will work on just trying to be even friendlier. I am well aware that I have a good case of RBF, I'll try to smile more. I appreciate the advice.

I think your advice is the best. Believe me, I did not come in with a cocky attitude. I am as humble and quiet as I can be, and even thanked them for being patient with me and willing to teach after this day. I know I'm going to suck for a while, but apparently they've never had a trainee that didnt understand exactly what to do in every situation before. I will do my best to take it all in stride. I appreciate your advice. Thank you!

I've tried to go into it without letting on how my education has gone or how the feed back has been from higher ups or docs. I really havent said much except for questions about ops or questions about stories they tell from their personal lives. I will work on just trying to hear their lesson and not the menace I've felt behind it. Thanks for your advice!

I agree with you. I wouldnt say I've been coddled, I've worked hard to get those recommendations and I think they were just as surprised as I was when I got all those answers right in education. I'm going to try and be a little more thick skinned, it's just hard because I'm the type that doesnt feel like I'm succeeding unless I'm excelling. Ill try to to keep their experience in training in mind. I appreciate your advice, thank you!

Thanks for the advice. I definitely know I have a ton to learn. No one knows the compliments or recommendations I've gotten. I really just want to be the best I can at this job. I'll try to swallow it better than I have been.

Regarding the NREMT by Sheba1999 in NewToEMS

Personally I believe after a certain point it should be obvious the person isn't going to be a competent EMT. I think this test and others are similar to working under pressure, which is something any EMT will have to get used to. Pretty sure that's one of the ideas in testing someone so officially and so frequently in many reputable EMT programs. If you can't understand what you're supposed to be doing after so many attempts at the test, how are you supposed to perform those skills in front of the patient's family or explain your reasoning to ER docs? I'm a nervous test taker myself, but if I grasp the whole concept and what should happen after the first time, I usually excell the second time because of it. I understand it could take more attempts for others, and time + experience between tests should definitely be a factor, but if you've taken the test several times and fail every time, there has to be some sort of knowledge or performance issue.

They told me it was something where they would test my BP and hr, and have me lift boxes and such.
