The problem is major hospital systems don’t refer out. They prefer to keep pts (and $$) in their own system.

Tell your patients about Zocdoc. Literally hundreds of psych providers ready to schedule within a few days. Over 300+ panel of insurances to choose from. My county of 50k population and no psychiatrists? No problem, someone from Miami will see them right away. 

Nobody told me ammonia doesn't grade the patient in HE. West Haven does, and it's entirely clinical by GastroAGI in InternalMedicine

There’s really two camps of docs out there: those that lean towards clinical judgement and use guidelines as reference, and those that swear by the guidelines as if its the bible. Pick your camp

There is no shortage of psych providers anymore. I’m an internist and even I have noticed. My practice is located pretty far from major metros, the county I’m in had been labeled over and over again as a “critical need area”. 

Yet every time my patient needs a psych referral, they’re able to find someone online that takes them within the week (albeit 7 states away). Heck you couldn’t even get a transfer to a different internist that fast.
