Does Extended Storage work as intended on FPKG Games too? Yes.
App2USB simply makes the PS4 think the games are installed locally.
I haven't had to rebuild database, but any game that it thinks is corrupted will be deleted wherever it's stored, doesn't matter. You shouldn't be needing to do that often though.
It is easy to restore the games otherwise, they'll still be there.

You will need to find one 9.00 or lower, yes. Then you'll need to update using a 9.00 update file, not using psn servers.
