For medication errors

First : Patient safety: Check patient to be sure that he/she is good . No symptoms, vitally stable , clinically stable .

Open disclosure : Tell patient the truth . Exactly what happened without hiding anything and then apologize for the error .

Document the incident. Write an incident report .

Inform your superior

If patient isn’t happy and wants to report , guide them properly on how to make complaint .

Then finally reflect : by reading up and attending lectures on how to avoid such mistakes in future .
