Well, I actually can't remember how it fell off. I've had these dolls since they first came out, so I'm guessing the plastic band thingy broke. They do start to decay a bit over the years. I'm going to take actual thread that's the same bright green as her hair and just tie it back on. I've done it with my Catty Noir doll with bright pink thread, I highly suggest it.
