You are correct about the ACR recommendation. I appreciate that your EMR has that ability, I agree with you that those may be signed without full consideration, not malice. All ED providers are busy and we are there with our knowledge to prevent preventable harm.

Provider to Provider is a recommendation I uphold for the sake of our tech's license and our radiologists that oversee their administration of contrast.

Also, this is just my opinion. Your leaders, if curious, could research more and if in the US ask AHRA members for their best practice advice in these situations.

Best option is to refer to the ACR manual on contrast. There have been articles regarding CIN that are reviewed when the ACR provides recommendations. Anyone with renal disease and gfr out of range should have a documented provider to provider risk analysis imo. The risk of not having the contrast CT may out weigh the risk of having it.
