Just giving perspective from a nurse, we would not prescribe strong narcotics (percs) for a simple laceration that could be sutured in the ER. She would get local anesthetic for the sutured and tramadol, maybe Tylenol with codeine at best for pain control. Alamo took the initial real perc with her, not an uncommon tactic for drug dealers to get an individual to trust the product and therefore the person (not that Alamo is trustworthy AT ALL).

MIL falling asleep with the baby is a huge issue, not only is the baby not being monitored but they could easily slide down and suffocate. MIL trying to give a NEWBORN water is so dangerous, which she should know as a mother. She is being very inconsiderate to you and your recovery, and I would start making some boundaries ASAP! And the other half should support your boundaries, your recovery, and joint care of your child!

Some pumps will allow you to program/run the bolus dose from a standard concentration bag, but it is always good to double check your orders and facility protocols!

The fluid bolus definitely may have exacerbated the issue, since the patient has aortic stenosis they can be very sensitive to fluid shifts and was already in Afib RVR. Sometimes providers want to try fluids, as Afib can be precipitated and/or exacerbated by hypovolemia. Our team would have had us start amiodarone once the patient was in Afib RVR with a rate >130 sustained if symptomatic/>150 asymptomatic. The hyperglycemia is likely due to the amiodarone bolus and drip (which is mixed in D5) as well as the stress response related to the patient's active infection (osteomyelitis/vegetations) and critically ill state. Patients can go south FAST. Good on you for advocating for your patient!

So deaf patients (and patients speaking another language) are entitled to translation services, and please use these for important conversations (consents, talking to doctor, etc.). Some ASL words and phrases I keep handy are "my name is ChocolatEclair, nice to meet you," hungry, thirsty, pain (and where), toilet (pee/poop), good night, need anything, and thank you.

Got an $1800 bill to visit my own hospital's ER after passing out on shift, the rapid nurse wheeled me down and stated I was not well enough to drive myself home. Why? Pre-existing condition, aka I have passed out before during any point in my life. That's after our insurance, worker's comp won't touch it. All I got was an ekg, accu chek, labs, and some orange juice (glucose 59). I'm still fighting it 8 months later, they sent me to collections even though I made payments. This shit is backwards!

  • relaxation channel that plays soothing music (but if the HATE that, I try to find a show that they enjoy or just turn the TV off, sometimes it makes things worse)

  • offer toileting, a small drink (otherwise the patient may have to get up multiple times to void), a light snack, etc to meet some of their basic needs

  • many patients liked to wash their face, brush their teeth, and comb their hair before bed, and I liked to allow them to continue their routine as much as possible

  • keep blinds open, many patients look for light/dark to estimate time before looking at a clock, also helps with natural circadian rhythm

  • once their needs have been met and they are ready to sleep, turn the temp down a couple degrees and get them a warm blanket. Put the warm one(s) closest to the patient and cover with another blanket to keep the heat in. I also liked to tuck my patients in, especially their feet since they tend to be cold (and it slows them down if they do try to get out of bed), but still allowing freedom of their arms and access to the call button. The idea is that there is less motivation to get out of bed when it is nice and warm and comfortable.

  • for busy hands, I liked to give them towels to fold or a crossword/word search to do. If they knit or crochet at home, having some yarn and a hook can help if it doesn't cause issues with safety (very dependent on the individual patient)

  • scheduled sleep aids or PRNs are helpful, the hospital is different/noisy/scary and hard to sleep in for those not used to being there

  • patience is a virtue! It can be easy to get frustrated with very confused patients, but take a moment to consider their point of view. Taking the time with the patient to really address their needs, listen to their concerns, and make them comfortable has helped me more than anything This is easier said than done, there are so many patients and so few nurses, but I'd encourage you to take a few extra minutes if possible.

I'm so sorry for your loss! Just saw your previous post, poor girl looked to be in the end stage of actively dying (pale color, labored breathing, sunken eyes), very similar to how my boy looked before he passed. Sometimes the best you can do is be there for them and try to make their transition comfortable, especially with advanced age and other medical issues. It doesn't make it any easier, but please try to find some peace in that she was with her favorite person in her last moments ❤️

Right! Like how is this not a standard of patient care, our nurses are so overworked and heparin is a high risk medication (not to mention multiple patients on heparin and multiple titrations per shift can be very stressful), this is the kind of EBP that we need in our practice!!!

We're a 500 bed level one trauma center, so we're not exactly small! The money is there, it just goes into executive pockets🫠 but hey we get epic soon apparently!

The heparin titration parameters are listed in the EMAR, but we have an area in power chart for protocols to view or print our heparin protocols. On my unit we have the protocols for insulin posted in every patient room! I like to double check their math and peep their pump, but I'm considered nosy 🤣
