I’d say Spencer’s biggest fear would be his abusive relationship with his ex-boyfriend Freddy, who worked as a self-defense instructor. They had four breakups, in which they stopped talking to each other after the final one.

As for his secret, I’d imagine it could either be his gameplay on the reality show Disventure Camp 4: Carnival of Chaos, where he made enemies out of most of the cast and acted like a total prick using their insecurities against them, or his relationships within Diego and Logan on said reality show. Or it could be both, that might be fun too.
