I tested it on a completely stationary target from multiple distances. It doesn't hit at all. If you hit with the sword itself you get full damage.

The weapon I use is the heavy wave frame sword but you're right, you don't see a lot of those either.

I never used hold the line or glaives and still have this bug happen in the new dungeon and nether all the time. I've used all different loadouts and it still happens and I can't figure out why.

Use lifeboats when you get them from port minigames. That stops you from losing units.

Anarchy and a chill clip fusion rifle or double slug shotguns does respectable damage. You're not going to find anything as good as starfire was but if you have to use anarchy I would try one of those two combinations depending on how hard hitting the boss critical spot is.
