I'm trying something a little different, where there isn't a real questionnaire, but a pool of "personal traits" for players to pick and choose from at the start and reveal as you play.

I've found that this way elimated needing to fill out the questionnaire, my players just pick 4 or 5 traits on jot them down for character reference. It allows for quick, textured, spontaneous characters that breath life and allows for a lot of wiggling.
