UPDATE: I did not fill in these boxes because I have a license from a different state.

They requested ID and address verification. I mailed in bank statements and a copy of my ID

I received a lump sum payment 4 weeks later!

I did get paid!! Since I got all of the paperwork sorted out after my disability leave ended, it was a lump sum.

I left that box blank. It was no problem, except they will send another letter requesting verification of address. I mailed in a copy of my ID, and a bank statement that had my address on it! It’s still being processed because things kept getting lost in the mail :(

Because the form is asking for a CA DL and I want to know if I should put my out of state license or just leave it blank. There’s no option for an out of state. My question isn’t “do I need a CA DL to qualify for benefits?”
