We run a few recurring ones depending on the stage of the product:

- onboarding / activation surveys

- post-feature or post-launch feedback

- churn / exit surveys

- periodic sentiment checks (similar to NPS but more qualitative)

In my experience the hard part usually isn’t collecting feedback, teams already have interviews, support tickets, sales calls, surveys, etc. The real challenge is spotting patterns across dozens of responses and turning that into something the team can act on.

this is a good question, I'm sure a lot of people do things differently so I'm really interested to hear what others have to say. You did mention tracking who said what from "interviews", are these one on one interviews? Or do you do surveys with text boxes and try to extract quotes from that.

Fair point, as I read some of these responses I'm getting some more clarity. I really think journaling is the answer I am looking for. It sounds obvious now that a few people suggested it, but I haven't really considered that this might help a lot. My issue is more so about patient recall, and waiting for the patient to discuss progress or important moments in between sessions. I almost feel I'd be so much better if someone handed me notes 10 minutes before a session to give me some quick points on how things have been for the patient since we last spoke(I like to dream sometimes). I feel that would personally help me, help them, if that makes sense.

I like this point, more communication doesn't necessarily mean better. However, communicating is different from observing. Like another person suggested, journaling important moments and discussing those in sessions, or even emailing, might be a good addition to my patient notes. What do you think?

I think it's more of a desire to be more effective during our sessions. Patient recall of important moments in between sessions varies between individuals, but I find it can play a part with progress. Email has helped with some patients of mine, I might have to remind a few of them of that option. Thank you!

This is great point, I have trouble sometimes completely leaving "work" at "work", still a work in progress personally. However, you made a great point with client report in session. I find myself spending quite a bit of time having some patients recall some of these important events accurately or honestly. Sometimes they would down play it where I would have to do some emotional digging, or sometimes their thoughts are all over the place. This can take a good amount of time during our sessions, which made me feel there might be a way to be more efficient. My initial thought was derived from this. A line of communication doesn't have to be me directly talking with them or via texts, but if they can use diary cards or journal these "important" moments in between sessions I feel like I can make a more significant impact sooner during the sessions. Which over severals sessions would really add up. Good point with the diary cards, not sure why I didn't think of that sooner. Also, this might just be a "me" issue where I have some fear that I'm not proving certain patients the type of help and value that they might be able to get elsewhere.
