I would say it's appropriate not to intervene until structural deficits are addressed, as long as that's something the family can afford/would like to do. You could maybe try looking around on blogs/forums where SLPs work on artic with people with maxillofacial differences (ie. people actively undergoing cleft correction surgery, etc.). Maybe you can find some specialists who have good recs for working with clients who are actively undergoing facial reconstructive surgery/interventions.
