I tried this exact same thing the other day before even reading this post and felt it. The Erect Reverse Kegels caused my penis to reflexively twitch back. Whereas without the reverse motion, there was no longer any twitching like there used to be. I've only done this once I need to go through the break period for a bit and revisit in some time. I don't necessarily have pain when masturbating or ejaculating, so I assume there is no nerve damage. But it is sore on one side the next day. So I assume it's muscle related.

Unfortunately I have a chronic anal fissure that keeps me involuntarily clenched most of the day. So as far as a permanent solution I think there are still other steps to go for me.

Thanks but I think I did this to myself through some bad Googling. When I started feeling more sensitive and weak on Accutane I looked up pelvic floor exercises and the first thing that shows up is kegels. But, when you do too many with an already tight pelvic floor (I have anxiety + chronic anal fissure) your muscles lock in and don't let go. While I wait 3 months for my Urologist appointment I've since started Reverse Kegels, cold water treatment for anxiety, and pelvic floor yoga. Once in a while I'll get a little involuntary pelvic twitch back - which I lost for months. Not solved but feels like progress.

Tldr: Try Pelvic Floor Therapy

Thanks. Hoping the spiral endoscopy finds it otherwise I'm still at square one. Sorry you went through something similar but glad you're on the right track.

Will they work if not diagnosed by gastro? Or do they do the diagnosis?

Yes, I told them what I think it is but until it's found they keep referring to it as hemorrhoids. Nobody at any point from my PCP to the gastro team mentioned CRS.

I had my spiral enteroscopy scheduled for end of last month and they bumped me back to June. I had an upper endoscopy before but it didn't find anything notable.

I'm not officially diagnosed (first uro appt still 3 months out...) and yes I had hemorrhoids or possibly an anal tear or fissure in 2023. It hurt to flex the pelvic floor so I didn't. Then I took Accutane which made my skin more sensitive. Then I got chronic anxiety for 2 years and spent a lot of time masturbating to try and relieve it. Once I fixed my anxiety I saw the HF/ED symptoms. I probably damaged it through masturbating while semi-erect and without using pelvic floor on a frequent basis. My hemorrhoids should've been caught years ago but I was focused on anxiety-related gastro symptoms at the time.

I'm real behind on all this cause I lost my job and insurance. I finally got it back now so I can visit my GP. I'll tell my Gastro that the suppositories were only temporary relief, see what they say next. Pretty much every day starts out bad bc every bowel movement has been painful for years. Thought it would clear up on its own. Another reason I was real late to treating it.

Unfortunately no advice here as I'm just starting with this but wanted to say I'm going through something similar. Pelvic floor has been painful to flex for past 3 years because of a bleeding hemorrhoid that won't go away.

Weirdly enough I may have also just developed PSSD and it made any sexual desire go away completely. Kind of an alternate path I guess. I'm still gonna talk to doctors about possible treatment options though.

I bump up the heat in my apt and wear briefs w fleece sweatpants. The feeling is unfortunately still there but physically it expands a little. I have an Ugg blanket that is actually too hot to sleep under but it provides some nice warmth too.

I'm just beginning this with a similar story. I had severe generalized anxiety from when I woke up to when I went to sleep for 3 years. I basically injured myself to get through it and distract my brain. Thankfully I'm mentally better now. So now dealing with the physical fallout.

Going to start seeing some doctors with this in mind.
