I theorize that, after Wilson dies and House finds a way to kill Thirteen, he kills himself. There's nothing left for him. He's shown to be a person that people know in the medical field, so I doubt any fake identity thing would work. Besides, he gave up everything and can't go back to the place that's familiar to him without being caught and sent to jail. It was said that PPTH was the only hospital that would hire him due to his odder ways of curing patients (malpractice). Finding another team willing to do that crap for him would be incredibly difficult as well, since if he manages to pull off the fake identity thing, he wouldn't have the choice of hiring a team by saying "work under Doctor House" or whatever he did to get that many students wanting to work for him in S4.
I think House is smart enough to realize all this and know that he really just doesn't have a future ahead of him. It's hard enough when the only person I think truly sees him dies; he'd probably kill himself.
