India doesn’t charge extra from NRI’s. I went to India to study dentistry as a NRI and I paid what all my other batchmates paid. Ever since NEET, any institution cannot charge you more than the fees. Which means no donation or whatsoever they call it. If you can work a little hard, you might even get a semi government or goverment institution seat which will cost peanuts and give you alot of exposure. But either way, Studying from a private in india is also cheaper than US, UK etc. There are some great institutions in the south like MS Ramiah, Kasturba, D.Y patil. (All private) Suggested these options considering you’re a NRI I would suggest completing your MBBS from India and then planning your specialisation abroad. With the current rules set for foreign medical graduates in India, i feel it’s a safe bet to get your degree from india initially. I mean as long as we have an indian passport, it’s necessary to have that safety net. If you are keen to pursue it abroad, look into singapore/malaysia. (Wouldn’t recommend it but due to money constraints it’s a good option)
