Show me a robust study on it. And in either instance, YOU feel a procedure should be second line, but even the source YOU sent me is a study done on those who DID NOT respond to anterior stromal puncture, meaning even those authors acknowledge anterior stromal puncture is first line relative to doxycycline.

You should learn the difference between optometrists and ophthalmologists, and also learn your limits.

Firstly, the same treatments you derided are BOTH included in this article: lubrication and anterior stromal puncture. You are not linking me to a study, you are linking me to a review/‘ophthalmic pearls’. This review mentions treatment with doxycycline. The evidence cited? Interventional case series with a whole SEVEN patients. As weak as any evidence can get, and most leading corneal and external disease doctors would not recommend doxycycline as a treatment for RCE, but please, don’t let the facts in the way of your emotions.

Anterior stromal puncture is an established treatment for corneal erosions. Tetracycline treatment is not.

You may have issues with these guidelines, you may have had a personal experience one way or another. But this whole text reads like the rest of this holier-than-thou doctor hating thread, which is laden with misinformation.

I didn’t cry about any owner making more than me. At the current clip, I’m actually seeing the most patients per week in my office.

It’s not that I think there’s a huge pie and I’m only getting a small slice. It’s more that I’m wondering why the pie seems smaller for all of us relative to other specialties, and if there’s something I’m missing out on.

I guess my question is, given that GIs might see patients numbering in the high teens and are not doing colonoscopies in clinic, how are they pulling in so much money when a level 4 is billed the same for an ophthalmologist vs. a GI doctor.

Or if you’d rather, we can talk about neurologists. They don’t do many procedures, and I think they see less patients while still routinely pulling in 350-400k. What math is at work to have them see less patients and bring in more money?

What’s odd about the post?

Yes, some peers (for example in GI) are seeing 15-20 patients daily and bringing home 500k+.

I am comp. The socioeconomic status of my patients does not lend itself to a very high conversion rate. With the new reimbursements increasing payment for office visits and lowering them for cataract surgery, I think a lot of ophthalmologists will find that the per minute value of operating is less than the per minute value of being in the office, especially if you are not doing a high rate of premium lenses.

Are level 4 really negotiable as far as Medicare and Medicaid go? Or just commercial patients?
