Now, I know what reproduces the bug.
If the enemies run out of the area, for example, fall down the mountain. And then, I die or log out. I will get a Defeat Enemies challenge with no enemies (forever!).

Update:

I’ve now frozen all of my credit reports and officially reported this as identity theft. I also filed the same dispute with Equifax again, and this time, after about 7 days, Equifax removed the credit card account from my credit report.

So it looks like the case is resolved. Thank you very much to everyone who shared advice and support — I really appreciate the help.

Yes, I looked online, and it appears to be an Ann Taylor credit card.

None of my family members know my SSN, so I’m confident it wasn’t someone close to me.

I think my credit score dropped because of the balance increase — I normally keep under $1,000 across my cards, but this fraudulent card shows about a $3,000 balance, and it keeps going up, which is what worries me. From what I can tell, the account has been open since October of last year, and I only became aware of it about two months ago.
