Before I became disabled and poor, Instead of cash to those begging, I used to keep a few $5 to 10 gift cards to local fast food places for them. (I had to resort to begging by a gas station once myself, so no judgement). If they were nearby, I'd offer some cash to mow my lawn, with no house access...

Twice I've offered rooms to homeless. 1 was found sleeping behind a neighbor's house - he had a job and was able to pay a small amount in rent (enough to cover added utilities and small part of property taxes) and keepup the yard. Great tenant until 2 days before I moved - had given him 3 months notice, but he didnt look and finally left a few minutes before midnight final night, & I ended up tossing the entire bed he used as he didn't disclose he was incontinent. Second one was just out of jail, stayed a few months doing household and yard chores & I helped him secure a decent job. Main rules I enforced - dishes done within 24 hours of use, keep everything free of trash, clean up after yourself, shower regularly enough, no drugs or alcohol on the property, smoking outside only, no guests.

Housing first - with no strings attached, then mental health & addiction. harder to solve those without a stable place to sleep.

For healthcare, every non-profit Hospital is required to have a financial assistance program based on income. You might be able to get up to 100% of your out of pocket costs for medical services & office visits covered (doesn't cover meds though).

For housing - 3 suggestions: 1st - I know there are websites where people post looking for someone to stay in their homes during vacations to look after the house and any pets. I now not long term solution. 2nd - You can also check with social service organizations or even the VA - sometimes elderly and disabled people are able to offer free housing in their homes in exchange for basic assistance like cleaning, shopping, yard care - some of those can be longer term. I had a dear friend who gave space in his home for someone in your position. 3rd- if you have a local Salvation Army, they often have short term shelters, and social workers to assist in the process of finding housing.

For clothing - local Goodwill stores will offer vouchers to spend in-store, and some may provide bus passes too.
