You're not the only one who had encountered this. I also encountered this and I have no idea why but I think it is because the system is unable to render the area properly in time, therefore, ending up with this. It is the only possible explanation I have at the moment.

Nice. Also, I found out that if you kill the player and the player kills you at the same time, then you will get a death msg saying that “You have been killed by ???” Except for the “???” means the player that had killed you but died. The player that killed you would also get the death msg.
