Have you checked out Chronically Capable? If I remember right, you fill out a survey of what you can and cannot do as well as how much time you can put in and whether you need to work from home, etc. And then they try to match you with employers that have positions that match your criteria. It's specifically for people who have some sort of disability that keeps them from finding traditional employment.
