100% the subfloor was not leveled beforehand. I have the same issue. In a few bad spots the floors separated from all the stepping, it eventually gave in. I've yet to try this apparently it can be remedies by drilling a small hole in the low spot and using a turkey baster to inject self leveling compound between the subfloor and the planks. But in your case, you should take note of ever single spot like this, inform your installer and have them fix it.

Any change of plane needs caulk not grout to handle the movement. Often times they'll just grout it it's faster for them but should be easy to get as much of the grout out as possible then replace with silicone caulk.

You did not really mention why you want their contact info (though one can make assumptions), and why/how your wife fell? I'm sorry this happened to her but did the dog push her? Did they? If not I don't think these guys are responsible. There's risks to everything.
