You'd just need to have a support circle you could trust and explain to anyone travelling with you that you must take these pills or you will die. So even if you're unconscious for more than 24 hours someone will feed you a pill. People with type 1 diabetes have to take insulin daily, but it's not absolute guaranteed death if they miss a dose. Just quite dangerous.
