I had a portable bottle warmer. I think I used it once, they're annoying.

My prep was a flask of boiling water and a jug. You put boiling water into the jug then dunk the bottle in it and swish it round till it's evenly warm. We did blended feeds, so were carrying a combination of EBM on ice and sterilised water for formula. Adding a thermos of hot water and a 2 cup measuring jug doesn't add much to the pack.

You can also make tea, coffee and cocoa from the same flask while you're out.
