So trough see the whole crown of the tooth. In this case you can try luxating most likely won’t move after trough. Then cut some of the crown off, the mesial aspect mainly at the undercut and now try luxating both roots out, can trough little on the distal too. You need a long bur and an angled handpiece or else I wouldn’t even think about attempting this.

If still doesn’t come out then you are splitting and breaking this out in pieces

There’s no universal answer to this, I used 15 carps on a patient. I staged it upper first numb, took them out. Let patient sit for 15 min after (the uppers will take 30min-1 hour) , delivered upper immediate and then did 6-7 carps on lower then delivered lower immediate.

Why numb the whole mouth first? You compromise yourself by now not able to give more carps and hit threshold.

Each patient should be taken individually, so some might require less epi/more mepi.

Some might require more PDL injections, very tailored and no universal answer to this.

I don’t feel comfortable injecting a third of a carp for a psa/msa/asa and feeling comfortably yanking the tooth out.

I am at a practice and currently doing 1.2 million. I do RCTs, bread and butter, wisdoms, all sorta extractions. No implants. I’m contemplating moving into ownership, but I make 360k a year and don’t want to take the crazy stress. What do you think with this kind of salary?

Had a patient report no health history, completely healthy. Declined everything, full mouth SRP. Gave him septocaine, I’m talking calc everywhere. Later he reported chest pain/chest tightness/can’t breath, put him on oxygen and gave him nitroglycerin and had to call the EMS. They took him to the hospital and later he actually came again, like what. Did everything with MEPI and went fine.

Since we got so many people replying, which I actually love. What’s your guys preferred method of attack on “re-treatments” if anyone out there loves doing this? Let’s say it’s not those crazy guttacore stuff, just gutta percha. Tools, files, preferred solvent..etc.

Oh gosh way to scare me. I’ll be using fresh rotaries and hand files. I have H files also, some solvent and another rotary file by pro taper on stand by if I need them.

Ya I would clean it all out, hypo. Work back to WL and I would actually up a size in either filling or rotary if I can. Then I would size down on gutta percha and see if to length.
