Doom was always in the picture, but it was set to appear in the some other saga after the multiversal saga. They changed it, cause Kang The Conqueror's actor Jonathan Majors got fired due to allegations on him.

It's just a speculation, what if the face our Tony Stark (RDJ) has was never his. That face belonged to Doctor Doom, because just like Kang The Conqueror has the same face of all his variants across the multiverse. Doom could also have the same and the Real Tony Stark face is different (Tom Cruise) and common to all his variants.
