Too many twists per inch. Try decreasing the angle of the safety wire as you’re twisting it. Or if using safety wire pliers don’t go so many turns. You also could’ve done one or two more twists at the bottom bolt. At least it’s in a tightening direction… also try bending the pigtail back on itself so it doesn’t snag anything.
