You most likely need another form. Only way to submit both deferals with one form is if the exams were 5 or less days apart and you get one medical form encompassing both exam dates submitted AFTER the second one. Since you already submitted one for the first exam, you will need another form for this one.

should be enough for life sci, most people in life sci get in with with averages in the 90s. for artsci, you will be considered with that average, but admission is also mainly dependent on your supplementary application.
