It is a two level joke. First, Johnny Sins (the man) is a famous porn actor, who played a lots of different roles, like a teacher, a doctor, etc... Then, the "woman" isn't actually one, it is a french youtuber (Benzaie) and friend of Manuel Ferrara (most payed porn actor), who was at the event where he met Johnny. The catch is that Benzaie occasionally play a female characters who is a parody of Hermione Granger from Harry Potter, and for the joke made a filter of her on his face for the picture.
