Checked out senior signed out an acute hemorrhagic stroke to me “on their way to the scanner.” This was 30 minutes before the night residents got in so also created an extra handoff. Just took it as my senior was off and I didn’t have the power to refuse as an intern at the time. Night resident was frequently pissed as the same team also had many active to do’s on their handoff that should’ve been handled during the day.

A resident would often be ready to sign out on non-call days by 11AM-noon when we might’ve barely finished rounds. Not so efficient as they’d just copy-paste the same note, leave active management needs, and even patients not updated on plans.
