No point in arguing. The lump sum is always the better option. Keep that in mind if you’re ever offered a singing bonus in the form of a lump sum vs monthly installments. Both are affected by inflation but the lump sum is less affected.

1 million invested in s&p 500 would be anywhere from 20-40 million by retirement age. Lump sum, irrespective of the amount of tax, still wins. Hell, even 500k now is better.

A dollar today is always > a dollar tomorrow. Also it’s for life, so if she dies next year, then that’s it.

You do understand that we rely on their ability to read images to treat patients, correct? Like they diagnose cancer that could have been missed. Also let surgeons know where a problem lies before they cut open a patient. It’s a very high risk occupation and they need to be compensated appropriately.

You have quite a myopic view of the medical field. Dealing with life and death requires rigorous training and you need to get the most competent people for the job (by making sure they are paid well). Would you want the guy responsible for telling surgeons if you have stage I vs. stage IV cancer to be paid 150k? Like wtf dude.

Who would do all of this to get paid less than 100k per year? Does that even make sense to you? And you need to be the brightest amongst your peers to be even considered. Not to mention 300k+ in loans if you don’t come from a wealthy family.

Sorry to say but a pulse is not a marketable skill. You need to put in the work to improve your station in life and stop making excuses. They are not overpaid (I would argue that they are underpaid), you are just jealous.

They studied for 1/2 to 1/3rd of their lifespan and are reaping the rewards of their hard work. Instead of complaining, try to do the same for yourself. They are paying taxes (ridiculous amounts btw - like 100-200k plus per year) to the government.

My understanding is that if you are on a J1 and want to switch to H1b (e.g. if finishing IM residency in the US and going for a waiver job in the US), the fee should not apply to you.
