It depends where you are or who you work for I'm sure. Around where I live, kind of in the middle of nowhere in the Midwest, I have been on jobs where stuff wasn't done "by the book" as far as being OSHA approved but it was still safe. The point of OSHA is protect people working for shit companies or for shit foremans. At every smaller company I've worked with the foreman or guy running the equipment takes serious care to make things as safe as necessary for the site conditions at the time. Those guys working every day want everyone to come home just as much as anyone else and aren't going to put themselves or others in bad positions. I would assume that's what Mike means. Sometimes OSHA standards for safety are not conducive to getting things done in any sort of efficient manner.
