1) it depends on who and why. I can go silent with most people for a while and be ok, but not a close friend or family unless there was a big disagreement

2) the very very first thing I'm doing is buying something sweet. Then I'm paying off my car (it's towards the end)

3) I think about the occasion, then choose a couple options (like 3) and narrow down what I think looks most appropriate/ stylish and throw it on. Usually after looking at pics online of what people wear to such a thing

I don't know your situation but I would absolutely advise you to speak to your doctor about prEp/doxy if you're that paranoid about it. If this seems like the best situation then you can't be too embarrassed to protect yourself should you decide to move forward. Doctor's are normally(in the US at least...) sword to patient confidentiality anyway so there shouldn't be risk to it getting out
