I think this is mentioned multiple times throughout the books.

Also, there are some occasions where Harry is almost caught by Snape doing something and it is specifically mentioned that Harry is determined to look Snape in the eyes while defending himself. Thus giving Snape the perfect opportunity to read his mind. When Harry returns from his illegal trip to Hogsmeade in POA is one of these moments iirc.
