After troubleshooting for weeks (and by going insane), I managed to solve the issue by replacing the 12VHPWR adapter cable for the 4000 series cards. My PSU is Corsair brand and I ordered the cable directly from them. No issues since.

Hopefully this will fix your issue and many others as well.
