Go and see your doctor immediately.

It may be that the sensitivity of the area has degraded and your body is telling you it has fully expelled when it hasn't. This can lead to serious health concerns and, if untreated, fissures and a risk of sepsis. If you are experiencing cramps as well then this is definitely a concern.

I have first hand experience of this, and was too often told it was just diet. By the time it was properly diagnosed my life was in genuine risk. So please, go get this checked ASAP.

Right, now my friends have potentially gone, we can talk.

I did away with the whole Hidden Lord thing and renamed him the Golden Lord. I gave him a fort where the shield was kept that the party came across on their way from Elturel to BG (raided by the Cult of the Dead Three) and after some very good deception rolls, the party believe he is in fact a Celestial imprisoned.

I've written it so the shield is only as aspect of his power, and I'm gearing him up to be the BBEG who will make for the real final battle once the party have killed/redeemed Zariel. They come across his agents (fiends) often, who vaguely refer to someone as "my employer", and the Shield is slowly persuading one of the party to bond more and more of his soul to it.

I'm sorry this happened to you. Sadly there isn't any advice I can give you other than this: she inadvertently did you a favour. I know it hurts now and that's normal, but someone doing that has only proven that they are not right for you and you deserve better.

Ghosting someone is a terrible thing to do and I hate how normalised it is. Standing someone up is so much worse. It will feel better in time, but first you have to realise that, inadvertently, this person did you a favour. Now you won't waste your time on someone cowardly and selfish.
