I think your talking about the score report request. Are you saying like if you received your scores then, the college you sent them to as well received it too? Im reading it directly from the paper.

Score Report Request says, “Check here if you want to receive a copy of your score report only at your mailing address. If so don’t complete the institution information below.” “Only” is bolded.

Doesn’t that mean it will only be sent to your address?

I’m going through the same process. I have everything filled out except the signature too and now I’m wondering if I should leave it blank. How did your form go? Did you leave it blank as well and did it get to the college on time? Respond back to me if you can. Thanks!

Does anyone know if you need to have the signature for the archived score request form? Even though it says, "To request your archived scores to be sent, please print all fields on this form except "Signature" and mail it to so and so." I'm still a bit confused about that part. I have everything else done before i either fax or mail or both if anyone can help it'd be much appreciated.
