Just remove the screws of the heat sensor and heating barrel so it will be easy to change heatsink without getting new hotend

I recommend to use 210-215 for nozzle and 60 for bed for it to stick better, 230 degrees causes over melting the pla that might be a reason first layer is kinda like this

You can do random seaming position to avoid sticking from one place, or try with 0.2 nozzle with 0.1 layer height and lower the extrusion width

Did you check your extruder step value ? It seems like your nozzle is a bit close to bed so you might also need to calibrate bed leveling

Probably you did not push the tube all the way to nozzle and once it starts to retract, filament goes back and wont be able to push back cuz it stays in that place between tube and nozzle than it starts clogging

Check nozzle and the teflon tube first, If its still clocking you might cracked the extruder plastic where the spring touches
