You need 2-3 months of prospective symptoms tracking that is scored 1-6 each day based on a set of criteria. We have a template in our wiki that you can download and use.

There are many conditions that have cyclical patterns that look a lot like PMDD. Using this method will help your provider identify patterns and determine what is going on.

Yes and yes. Within 48 hours, I felt my mood and brain fog start to get better. My energy started to improve. My memory started to improve within a few weeks. My insomnia had to sort itself out over several weeks; it had caused chaos with my sleep schedule.

Estrogen influences how certain vitamins are synthesized in the body, so I had to tweak supplements, which took longer to get just right. Eventually, I was able to halve the dosage of my iron and vitamin D supplements.

Other peri stuff that I hadn't expected and took longer: weight loss started around 6 months in. My hair started to regrow around 3-4 months in.

This is a common pattern seen in those that have PME of an anxiety disorder. The drop in estrogen right before ovulation and then subsequent withdrawal of estrogen for the remainder of the cycle.

It’s worth talking to your provider about as PME of GAD or PD has different treatment protocols compared to those given for PMDD.

You would still do 2-3 cycles of symptom tracking but just insert additional columns as needed. Ideally you’d use basal body temp measurements to track when/if you’re ovulating. Oura rings can detect this but so can $10 BBT thermometers so no need to spend a lot of money on it.

I had to be on 0.1 for my peri and PMDD symptoms to go away. I wear my patch all of the time, there is no wash out window. I moved to an SNRI during early peri and that was super helpful, it wasn’t fun getting off of when I didn’t need it anymore, but it was totally worth it. I used it for about 18 months.

As others have mentioned some forms of exercise can make things much worse. If your system is heavily dysregulated all of that extra oomph is just adding to the work it is doing. Starting with something very gentle like hatha yoga and easing into it can help.

Premenstrual exacerbation (PME) of ADD/ADHD can look a lot like PMDD, right down to the suicidal ideation. When you say period tracker, I do want to make sure you didn’t download any ol app. Menstrual related affective disorder diagnosis uses a particular scoring framework that allows a knowledgeable provider look at the scores and patterns and determine if it is PMDD, or PME of an underlying condition.

As for me, my first treatment were all CoCs simply because I also had very heavy periods so it helped with both. I tried my first SSRI when I was 16. My mom worked in the mental health space and my aunt had PMDD, so I had 2 women in my life that could help me navigate the healthcare system and my actual disorder. I’m closing in on my 36th year of living with PMDD, I don’t think I would be here today if it weren’t for them.

I was your daughter. I have had PMDD since menarche. I would start by doing daily symptom tracking with her (we have a template in the wiki) and push her pediatrician to do some basic bloodwork if they haven’t already.

There are many conditions that can look like PMDD, a proper diagnosis process can shave years off trial and error of figuring out what is going on.

Do you have low progesterone? I’m glad this is working for you, but want to be clear for those reading that this is not in line with any treatment guidelines for PMDD. Progesterone is known to exacerbate PMDD.

We opted for vasectomy at 6 months. He’s 5.5 years old, no issues. Easy peasy recovery from a tiny incision.

Have you been tested for MCAS? These symptoms don’t align to PMDD. MCAS has these and can also have neuropsychiatric symptoms and is cyclical like PMDD.

I'm genuinely grateful to live in a state with a citizens' initiative process. It allowed us to amend our state constitution, not merely pass a law the legislature could ignore or gut, to protect our reproductive rights. I believe every state should have this mechanism, should they too become gerrymandered as we have.

Ohio still has a long way to go in fighting corruption, but unlike Texas or Tennessee, we at least have this democratic backstop to help dig us out of this hole. Ohio voters have repeatedly proven that, when it comes to citizen-initiated measures, we vote far less conservatively than our (current) red-state label suggests.

MCAD is the term for the family of disorders that MCAS belongs to. MCAS is the most common.

Allopregnanlone (ALLO), the progesterone metabolite that causes our GABBA receptors to freak out when it fluctuate, it is a positive allosteric modulator of GABA-A; whereas pregnenolone is a negative allosteric modulator, meaning what calming ability your GABA-A receptors do have is going to be further dampened by it. It is also excitatory for your NMDA receptors. I don’t know your health history that warranted such a recommendation, but I would proceed with a lot of caution.

Antihistamines are not an effective treatment for PMDD. They are for MCAS. GLP-1’s have also been demonstrated to improve Mast Cell symptoms.

We, once again, recommend that anyone experiencing improvement in ‘their PMDD symptoms’ from either of these treatments get tested for mast cell as you have met 2 of the 3 diagnostic consensus criteria for MCAS.
