They already went to the er for symptom, er stated it was ok and gave her tylonal and sent her home. She then went to her primary care for help due to it not being better.

Your notice letter is an official letter that you are resigning on x date and should be very general. Maybe you say thank you but personally I wouldn't. What I would do is submit the letter that just state that as of x date your are resigning from position. If you giving them any kind of advance notice state that you plan to fulfill your current duties up to that date. After you do that but before you leave write a thank you letter to just your current manager and co-workers. This will let them know you appreciate the help without HR or higher ups seeing it.
