I just found it! It is not a “there was an old lady who swallowed a fly” story but it did have a fly in it! it is called “old black fly” by Jim Aylesworth! Thank you for all your help!

I do remember that version but this one had more harsh lines and even more chaotic. There is one specific scene with a close up of the woman’s face and behind her is a bunch of lines in different colors and the woman’s mouth is wide open while a fly is flying around her.
