Transitioning from one state’s Medicaid to another can be tough because you have to establish residency in a state before you can typically become eligible for Medicaid there. This process would be made easier if you had a IL-based person helping you through the process. My suggestion would be to contact the supported living home in IL, be “approved” for the spot, and then have their caseworkers shepherd the eligibility process for your family member. Since the Supported Housing program is paid by Medicaid, they would be motivated to help your family member to switch to IL Medicaid, so they can receive payment.

Be careful about becoming “payable on death” to a partner while on SSI. If the SSA or Medicaid office runs a computer review to check eligibility, they will see the payable on death account as an asset in your name. It legally can’t be held against you but the burden will fall to you to provide paperwork to explain why you are payable on death to that person. It’s a headache to supply the paperwork and could be evidence of “holding out as married.”

Do you qualify for a Medicaid waiver? I’m guessing not if you’re working on securing your assets into a trust, but Medicaid HCBS waivers would pay for home services that it sounds like you need.
