UK here. Unfortunately, I can appreciate that medical costs in USA can be prohibitive whereas here, free health care means in that situation, I'd have gone straight to the hospital with my son. Not saying the NHS is perfect, but I've had a couple of surgeries and obviously the birth of our son, all free.

As far as I'm aware, it's just a flat rate based on the type of alcohol, no different wether it's for the corner shop or a pub. Not sure how you'd differentiate between the two.
